Periodic home visits, based on a physician's order to provide skilled nursing care, personal care, occupational, speech and physical therapy, social services, lab tests, diet instruction and certain medical supplies.  Patients enrolled in this program must be under the care of a physician and must meet the criteria for coverage established by their insurance, Medicare or Medicaid.

Services provided to Medicaid eligible patients as an alternative to nursing home placement.  Services for those who meet admission criteria include: personal care, homemaking services, case management and respite care.
