5 A Day Challenge Contest

The 5 A Day Challenge Promotion is open for entry February 1, 2004 to February 28, 2004 at 4:00 PM.  This contest is open only to residents of Kentucky and those 5 years old or older.  Those 18 or under must have the signature of a parent or guardian to be eligible.  The contest is sponsored by the Purchase District Health Department and the Kentucky Cancer Program.  Void where prohibited or restricted.  All federal, state and local laws apply.

Prizes: One grand prize will will receive $250.  Two second place winners will receive $100 each.  Three third place winners will receive $50.00 each.  Four fourth place winners will each receive a Veggie Character Book.  Five fifth place winners will each receive a 5 A Day Cookbook.  Six sixth place winners will each receive a 5 A Day Apron.  Seven seventh place winners will each receive a 5 A Day Tote Bag. 

Prize Restrictions: Winners must pay all applicable federal, state, and local taxes on prizes.  Non-cash prizes are not redeemable for cash.  No transfers or substitutions of prizes is allowed. 

How to Enter:  Go to the 5 A Day Challenge Calendar on this website, print the page and fill in all the information requested, including name, address, telephone number and mail it to 5 A Day Challenge Contest, Purchase District Health Department, 320 North 7th Street, Mayfield, KY  42066.  Only properly completed official entry forms accepted.  Entries become the property of sponsors and will not be returned.  Sponsors are not responsible for lost, late, stolen, incomplete, illegible, damaged or misdirected entries.  Odds of winning depend upon the number of entries.  Contestants are not required,  but are encouraged, to eat 5 servings of fruits and vegetables for 7 consecutive days to be eligible for the contest drawing. 

Prize Drawing:  Prize winners will be chosen by random drawing from among all qualified entries received.  Only one winner per household.  Winners will be notified by telephone.  Contest officials will attempt to notify winners the day of the drawing.  If a contest representative is unable to speak directly with a winner within seven days following the drawing, an alternate winner may be selected by random drawing.  all winners must sign a certification of eligibility and release, described in the eligibility section below.  Failure to complete the certification without making changes will result in disqualification, forfeiture of any further chances at prize, and selection of an alternate winner.  Any alternate winner must meet and comply with all the eligibility requirements set forth in these rules.

Eligibility: At the time of entry, entrants must be legal residents of the State of Kentucky as determined by valid government-issued photo identification showing name and address if 18 or over.  You must be a resident of Kentucky to win.  You must be 5 years old or older to be eligible to enter the contest.  Those under 18 years of age must have the signature of a parent or guardian on the entry form in order to be eligible.  Employees of the Purchase District Health Department and the Kentucky Cancer Program and the immediate family or household members of such employees are not eligible.   "Immediate family" means spouse, parents and children only.  Winners must sign, without making changes, a certification of eligibility and release which, among other things, releases sponsors and affiliates, agencies, subsidiaries, related companies, successors, assigns, agents, officers, directors and employees from any and all liabilities, claims, demands, actions or causes of action of any kind whatsoever for injuries, demands or losses of any kind, including those to person or property, including death, due in whole or in part, directly or indirectly, from participation in this promotion or the receipt, ownership or use of any prize, even if caused or contributed to by the negligence of Sponsors.  by participating in the 5 A Day Challenge Promotion, all winners are also voluntarily consent to the use of their names, photographs, likenesses, biographical information and voices, without compensation, in this and future promotions and marketing, except where prohibited.

Prize Redemption: All winners 18 years and older will be required to present a valid government-issued photo identification and proof of age and residency and provide taxpayer information, including social security number, before claiming prizes, and may be required to sign other legal documents.  Failure to produce or sign any of these documents will result in disqualification and forfeiture of the prize.  Winners under 18 years of age must present a birth certificate and proof of residence of parent/guardian.
